DIAGNOSTIC MEDICAL TESTING WORKS IN CONJUNCTION WITH NPF, INC. TO PROVIDE A FREE OXYGEN QUALIFICATION SYSTEM TO THE PHYSICIAN OR THE MEDICAL EQUIPMENT PROVIDER There is no charge to the physician's office or DME for the use of this system. There are no equipment leasing fees, annual fees, setup fees, technical support fees, cancellation fees, or any other fees. The physician's office or DME is under no obligation or contract to use the NPF System exclusively or for any specific duration.

NPF, Inc.: FAST, ACCURATE, AND TAMPER PROOF TEST RESULTS
With NPF IDTF Software, the IDTF must review the test results before they can be printed. NPF IDTF Software also highlights areas which may be considered aberrant data and provides the IDTF the option to delete these areas in order to prevent false qualifications.
NPF does not allow instantaneous reporting, nor does NPF create reports until the IDTF has reviewed the results. IDTFs must review each test and edit areas of artifact or aberrant data before they can create reports, eliminating the possibility of inaccurate readings due to low profusion, motion artifact, patients wearing nail polish, etc.
Because of the necessity for a hands-on review of each test, IDTFs are given 48 hours to provide the reports to the physician and the DME. In most cases, reports are available within 4 to 6 hours once the DME has transmitted the test and faxed the required forms to release the test results. Medicare guideline states that the prescribing Physician must receive the test results before they can be forwarded to the DME / HME.

TESTING CHECKLIST
MAKE SURE THE FOLLOWING STEPS ARE COMPLETED TO ENSURE A SUCCESSFUL TESTING EXPERIENCE:
BEFORE THE INITIAL VISIT TO DROP OFF THE UNIT:

Did you collect the following paperwork? Completed doctors order form and patient demographic form. (Did the patient sign both areas of the patient demograhics page?)

Do you have the correct patient instructions for the oximetry machine you will deliver?

Do you have fresh batteries in the oximeter?

Did you check the date and time in the oximeter?
WHEN YOU DROP OFF THE MACHINE TO THE PATIENT:

Give the patient the instructions, patient test diary, assignment of benefits and medical release to fill out and sign for their self administered test. (The test can not be released without this form being completed and signed by the patient)

Make patient aware of our toll free phone number so we may instruct them if they are having any testing difficulties.
WHEN PICKING UP THE MACHINE FROM THE PATIENT:

Do you have all the required paperwork and is it signed? (Patient Test Diary, Assignment of benefits and Medical Release) NOW ALL ON ONE FORM!!

Is there at least two hours of activity data on the patint test diary?
WHEN SUBMITTING THE TEST:
- Did you fax the order form signed by the physician or the order form accompanied by the prescription for testing?
- Did you download the oximeter through the NPF software?
- Did the test contain at least two hours of data? Did you submit the test within ten days of the patient self administered test?
- Did you receive the confirmation that the test was transmitted successfully?
ONCE ALL ABOVE ITEMS ARE COMPLETED YOUR TEST RESULTS WILL BE FORWARDED WITHIN 4 to 6 HOURS FROM TRANSMISSION. IN MOST CASES WITHIN 24 HOURS OR LESS DEPENDENT UPON VOLUME.
